Biodegradable vascular stent with enhanced multilayer surface coating for improved anti-inflammation, anti-restenosis, and re-endothelialization

Cardiovascular disease, causing over 17.9 million deaths annually, is often treated with biodegradable vascular scaffold (BVS) implantation. Our study highlights the importance of distinct coatings for endothelial and smooth muscle cell layers to improve BVS efficacy and reduce adverse effects like restenosis. Our research highlights the importance of differentiating cell layers when applying particles and drugs to BVS coatings. We propose a multilayer-coated stent: the luminal layer with α-lipoic acid (ALA) for antioxidative and anti-inflammatory effects, and the abluminal layer with magnesium hydroxide (MH) and everolimus (EVL) to inhibit platelet activation and prevent thrombosis. Doxorubicin (DOX) is used for its oxidative stress-reducing and apoptosis-inducing properties to enhance vascular healing. This stent effectively inhibited inflammation, restenosis, and promoted re-endothelialization in both in vitro and in vivo studies.
